We offer publicly funded vaccines (measles, influenza, tetanus booster, pneumonia vaccine, COVID vaccine) as well as non-publicly funded (Shingrix-shingles vaccine, RSV, TwinRix-hepatitis vaccines, etc.)

A person can submit this FORM (to MB Health) when they are looking to:
- Submit immunization records from out of province/country to be added to their Manitoba Immunization record
- Request corrections on their immunization record
- Request a printed copy of their immunization record be mailed to them
